A Vision of Root Cause Analysis: How Do You Rate? What is your vision for Root Cause Analysis? What would it look like at your site when you have "arrived?" This is the question that was asked of 850 people from many kinds of industries from all over the world. Although the specifics were different, depending on the industry, there was a common thread that ran through the comments. The comments came solely from industrial and business employees -- people admittedly passionate about Root Cause Analysis, but people who have other responsibilities asides from RCA. Root Cause Analysis consultants were asked not to add to the dialogue. In other words, the following comments are from practical, real-world, employees, just like you. The results are eye-opening!
