1.  Most people are ASHAMED WITH FAILURE.  They do their best to ignore it, sweep it under the table, and hide it.

2.  Our tendency is to SOLVE problems as fast as possible.  We very seldom take the time to truly understand their causes.

3.  We'd much rather FIGHT FIRES than prevent fires.  Firefighting is fun, rewarding, and exciting.

4.  We like to BLAME other people and things for things that go wrong.  Very few people are willing to LOOK AT THEMSELVES.

5.   When we live with the same problems every day, they become INVISIBLE to us.  When made visible by an outsider, we get very defensive.

6.  There are certain things in every organization that we know are not right, i.e., obsolete systems, cumbersome organizational structures, unwieldy communication chains, etc.  But these issues are so big, NO-ONE IS WILLING TO TACKLE THEM.
