remedies. Our
underlying premise is that "none of us is as smart as all of
us,"
and that no single method or person is capable of revealing everything that could be
revealed when investigating a failure or incident. On the contrary, it is
the variety of people who engage in open discussion that stimulate the necessary
challenge, questions, and debate that fill the gaps in our inherently
limited perspectives. Everyone
- both users and providers of
root cause analysis disciplines are encouraged to contribute their
thoughts. Those who are wise will be open
enough to learn from the ensuing dialogue. Whereas some will become more entrenched in
their own ideas, others will become inspired by others' viewpoints.

to limit the extent of "spamming," this site's membership
roster will not be made available for public viewing, nor will it be
sold to any user or provider. If users wish to be made part of a
provider's mailing list, the user will have to request such from the
